1Clinical Appropriateness

The Discovery Call is a collaborative conversation designed to determine whether an EMDR Intensive is the most appropriate treatment option for your goals, current circumstances, and clinical needs.

EMDR Intensives are not appropriate for every individual or presenting concern. At this time, Five Stones Therapy does not offer EMDR Intensives for personality disorders, bipolar disorders, or religious trauma. In some cases, traditional weekly therapy or another level of care may be a more appropriate recommendation.

Following the Discovery Call, the practitioner will determine whether an EMDR Intensive is clinically appropriate based on professional judgment, safety considerations, scheduling availability, and the client's treatment needs. When appropriate, referrals to other qualified providers or services may be offered.

2No Guarantee of Acceptance or Outcome

Scheduling a Discovery Call does not guarantee that an EMDR Intensive will be recommended, scheduled, or accepted. Every recommendation is based on the practitioner's clinical judgment and the information available at the time of consultation.

Participation in an EMDR Intensive does not guarantee any specific therapeutic outcome. Every client's experience is unique, and progress depends on many factors, including treatment readiness, active participation, personal history, and circumstances beyond the practitioner's control.

3Financial Responsibility

EMDR Intensives are private-pay services and are not billed to insurance.

• A non-refundable deposit equal to 50% of the total Intensive fee is required at the time of booking to reserve your scheduled dates.
• The remaining balance is due no later than seven (7) calendar days prior to the first scheduled day of the Intensive, unless otherwise agreed upon in writing.
• Failure to submit the remaining balance by the due date may result in cancellation of the scheduled Intensive and forfeiture of the deposit.
• Payment for services reserves professional time and clinical expertise. It does not guarantee that a particular therapeutic result will be achieved.

Clients are responsible for all fees associated with their scheduled services.

4Cancellation and Rescheduling

Discovery Calls

Discovery Calls may be rescheduled or cancelled with at least 24 hours' notice at no charge. Discovery Calls cancelled or missed without 24 hours' notice may not be rescheduled.

EMDR Intensives

Because EMDR Intensives reserve a significant amount of dedicated clinical time, the following policies apply:

• Deposits are non-refundable regardless of the reason for cancellation.
• Requests to reschedule an EMDR Intensive will be considered on a case-by-case basis and are subject to scheduling availability. Rescheduling is not guaranteed.
• If the client cancels a scheduled Intensive fewer than fourteen (14) calendar days before the first scheduled date, the full Intensive fee may be charged.
• If Five Stones Therapy cancels or reschedules an Intensive for any reason, the client will receive a full refund of all payments made, including the deposit.

5Identity Verification

A valid government-issued photo ID is required prior to your Discovery Call. Identity verification helps protect client privacy, prevent fraudulent scheduling, and ensure compliance with professional licensing requirements.

6Confidentiality and Practitioner-Client Relationship

Information submitted through this website, the inquiry form, and the pre-interview questionnaire will be treated confidentially in accordance with applicable federal and Ohio privacy laws.

Completing an inquiry, submitting the pre-interview questionnaire, or participating in a Discovery Call does not establish a practitioner-client relationship. A therapeutic relationship begins only after both parties mutually agree to proceed and all required intake documentation, informed consent forms, and financial agreements have been completed.

7Emergencies

EMDR Intensives are not crisis services and are not appropriate for individuals experiencing an acute psychiatric or medical emergency.

8Technology

Discovery Calls are conducted using a secure, HIPAA-compliant telehealth platform. Clients are responsible for having access to a private location, a reliable internet connection, and a compatible device for virtual appointments.

9Limitation of Liability

To the fullest extent permitted by Ohio law, Five Stones Therapy, LLC, its owner, and its agents shall not be held liable for any indirect, incidental, or consequential damages arising from or related to the services provided, including but not limited to emotional distress, loss of income, or interruption of daily activities. This limitation applies regardless of the theory of liability.

Nothing in these Policies & Terms limits liability for gross negligence, willful misconduct, or any liability that cannot be excluded or limited under applicable law.

10Governing Law

These Policies & Terms shall be governed by and interpreted in accordance with the laws of the State of Ohio, without regard to conflict of law principles. Any disputes arising under these terms shall be resolved in the courts of the State of Ohio.

11Website Information

The information provided on the Five Stones Therapy website is intended for educational and informational purposes only and should not be interpreted as medical, psychological, or mental health advice. Viewing this website, submitting an inquiry, or communicating through this website does not establish a practitioner-client relationship.

12Agreement

By submitting the pre-interview questionnaire and scheduling a Discovery Call through our scheduling platform, you acknowledge that you have read, understood, and agree to these EMDR Intensive Policies & Terms.

13Changes to These Policies & Terms

Five Stones Therapy, LLC reserves the right to modify these EMDR Intensive Policies & Terms at any time without prior notice. The version in effect at the time services are scheduled will govern the client relationship. The most current version will always be available at fivestonestherapy.com.
